    • Abnormal? Strain?

      • Can be either, depends on the patient's age. EKG has mild sinus arrhythmias (best seen in lead V1-V3), a bit prominent J waves (best seen in inferior leads), as well as generally high voltages (inferior leads, as well as chest leads), and an relatively vertical axis (between 60-90 degrees). All these findings can be normal in younger patients, which is probably the case here.

        • Abnormal. High viltage contractility all over. It could be systemic resistance such in HTN or increased contractility such as in increased preload

          • That’s very interesting to know. Thank you fir sharing this info.

            • Although it is not always the case, in typical LVH (usually due to HTN), you see a large R wave in aVL, reflecting leftwards axis. In this case aVL is negative, indicative of an axis of more than 60 degrees, possibly between 75-80 in this case (relatively vertical). The relatively larger voltages in chest leads (V1-V6) along with this vertical, or even rightwards axis, can be seen in many otherwise healthy younger patients.

              • Absolutely love this. There is very much to learn about normal heart that we didnt study in medical school.
